People We Serve

In addition to serving some private payer clients Everbloom home health care serves Minnesotans with disabilities or chronic illnesses who need certain levels of cae and qualify for the state's home and community-based waivers programs such as: 

​

• Community Access for Disablity Inclusion (CADI) waiver: for people with disabilites who need the level of care provided in a nursing facility. 

• Elderly waiver (EW) : for people over the age of 65 years who need the level of care provided in a nursing facility.

• Brain Injury (BI) Wavier: for people with traumatic brain injuries who need the level of care provided in a nursing facility that provides specialized (Cognitive and behavioral supports ) services for people with brain injury or neurobehavioral hospital level  of care.

• Assistance with arranging Doctor's appointments and transportion.

• Resources information and referral 

• Arranging community outings 

• Arranging needed social services 

• Socializing one on one and groups

• Round-the clock superviosn with nightime personnel on duty

• Assistance with bathing, grooming, nobility and dressing. 

• 3 nutritious meals and 2 snacks daily 

• Mdication set up and supervised medication administration

• Housekeeping, laundry, home maintencance

• Registered nurses available up to 24 hours a day 

• Privat and shared bedrooms.
